The Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include but are not limited to administration of medications, documentation of medication administration, and providing resident care while demonstrating the Sunrise Mission: to champion the quality of life for all seniors, in accordance with laws, standards, and Sunrise policies. Job Overview: The Medication Care Manager is responsible for providing high-quality care and services by administering medications and treatments to residents, documenting administration, and supporting resident care in line with Sunrise policies and regulatory requirements. Responsibilities & Qualifications

Essential Duties

Provide the highest degree of quality care and services by administering medication and treatments in a safe, organized manner. Review, read, notate, and initial the electronic health record to document pertinent resident information. Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse. Administer, assist with, and observe medications and treatments using the medication administration record and the Six Rights of Medication Pass (Right resident, Right medication, Right dose, Right time, Right route, Right to refuse). Ensure medications are passed on schedule using the mobile med cart; document and initial administration and report refusals or missed doses. Maintain confidentiality of all resident information and medication records. Report resident concerns and unavailable medications to the RCD or Wellness Nurse. Restock medication cart after passes; assist in checking medications regardless of packaging system. Assess residents to determine need for “as needed” medications and document/report appropriately. Count narcotics with another Medication Care Manager or Lead Care Manager each shift and report discrepancies. Maintain and clean the Medication Room, carts, and treatment carts; manage availability and disposal of medications, including expired items. Follow refill processes and notify RCD or Wellness Nurse if a new prescription is needed. Help residents maintain independence, dignity, and safety in accordance with Sunrise Principles of Service. Respond to each resident with empathy, considering individual communication patterns, history, and needs. Practice standard care precautions and infection control techniques. Resident Care Communications

Notify RCD of resident and/or family concerns. Attend and actively participate in daily Cross Over meetings led by the LCM. Participate in the development of Individualized Service Plans (ISP). Risk Management & Safety

Ensure compliance with national/provincial regulations and Sunrise safety policies. Follow PPE, fire safety, SDS, and Lockout Tagout procedures; report accidents/incidents and unsafe conditions immediately. Maintain safe equipment and workflow, including proper storage of oxygen tanks and medical supplies. Adhere to infection control practices and disposal procedures for bio-hazard materials. Training & Team Contribution

Collaborate with team to achieve goals and uphold Sunrise Credo and Principles of Service. Engage in engagement programs and annual trainings; attend meetings as directed. Maintain compliance with required trainings and certifications. Perform other duties as assigned. Core Competencies

Resident-centered decision making; calm under pressure; ability to manage multiple priorities; strong written and verbal communication; good judgment and problem solving. Experience & Qualifications

High School diploma/GED; may require state/provincial certifications. Maintain required certifications (e.g., CPR and First Aid) as applicable by jurisdiction. Must be at least 18 years old; previous experience with seniors preferred. Desire to serve and care for seniors; drivers may need to sign the Driver job description and comply with safety requirements when operating Sunrise vehicles. About Sunrise
