Location: Chicago, Illinois Business Unit: Rush Medical Center Hospital: Rush University Medical Center Department: Kidney Transplantation Work Type: Full Time (Total FTE between 0.9 and 1.0) Shift: Shift 1 Work Schedule: 8 Hr (8:00:00 AM - 4:00:00 PM) Pay Range: $43.60 - $78.04 per hour Rush salaries are determined by many factors including, but not limited to, education, job-related experience and skills, as well as internal equity and industry specific market data. Summary The Clinical Nurse Navigator will provide care coordination, follow up, and support to patients in an effort to improve their health outcomes. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures. Responsibilities Maintains and ensures compliance of all policies and procedures that guide and support the assigned services, including Joint Commission requirements, federal and state regulations, and regulatory standards. Provides consultation in specialty area to health care providers throughout the organization, affiliated schools of nursing, and the community. Offers guidance and emotional support to patients regarding decisions on follow-up and treatment plans. Arranges appointments with patients, reviews test preparation procedures, and assesses potential patient barriers to ensure the patient completes testing process. Contacts patients via phone, email, and letters to remind them about appointments, assists in scheduling/rescheduling, and answers questions about testing and treatment. Performs follow-up phone calls to patients after appointments as indicated. Works with participating sites to develop mechanisms for sharing patient data for follow-up, using existing data to optimize time and effort. Completes all necessary electronic records and paperwork regarding patient interactions and appointments. Develops and maintains relationships with community leaders, organizations, health institutions, schools, daycares, etc. Attends community-based events and health fairs; assists with forums, events, and presentations to educate the community. Supports reporting and paperwork for funding and grant requirements, including tracking patient metrics such as no-shows and lost to follow-up ratios. Tracks outreach and navigation activities and provides monthly reports. Demonstrates accountability for professional practice and development. Administers nursing care to patients when applicable, coordinating with physicians. Provides orientation/education for staff, students, and partner organizations as requested. Qualifications
Current State of Illinois Registered Nurse licensure required. Bachelor’s degree in Nursing or Master’s degree in Nursing required. At least three years of registered nurse experience required. BLS certification required. Excellent written and verbal communication skills. A high level of interpersonal skills is required. Proficiency with the Microsoft Office Suite. Data entry skills with attention to details. High degree of accountability and reliability. Ability to adapt to change and interact with a variety of providers with different protocols and care processes. Ability to work independently as well as to integrate as part of a team and clinic flow. Cultural competency and experience working with diverse populations, including ability to work in low-resource environments. Resourceful, resilient, innovative problem solver and motivated learner. Friendly, cooperative, passionate and compassionate. Respect for each individual's right to participate in health care services and decision-making; respects confidentiality.
