Amerita, Inc. is a leading provider in home Infusion therapy. We are looking for a Pharmacist to join our Pharmacy team as we grow to be one of the top home infusion providers in the country. The Pharmacist will report to the Pharmacy Manager and work in our City, State branch. Our Company Amerita is an entrepreneurial-founded company and a wholly owned subsidiary of PharMerica. The home infusion market is positioned for rapid growth driven by the aging population, increase in chronic diseases, robust pipeline of infusible drugs coming to market, and an industry shift from hospital delivery settings to lower-cost, high-quality alternative providers such as Amerita. As a core member of the Pharmacy team, you will be expected to perform pharmaceutical care activities for customers and patients within the service area. Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals. Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice. Adheres to all appropriate standards of accreditation and participates in quality assurance activities. Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution. Reviews, receives and verifies all new patient orders and documents on the respective order form. Assists the physician and other involved healthcare professionals with planning and implementing patient care in the home, setting therapeutic objectives for drug therapy, establishing monitoring parameters, and preparing regular progress reports. Conducts and documents initial and ongoing assessment of patient, drug and disease information to identify real and potential drug-related problems or needs. Monitors outcomes of drug therapy by collecting, evaluating and interpreting laboratory data, clinical data and other pertinent patient information. Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use. Maintains thorough documentation in the patient record of all clinical activities and communications with patients, physicians and other healthcare professionals. Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy. Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences. Compounds sterile mixtures and non-mixtures (e.g., parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics) using aseptic technique where state laws permit. Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies, USP 797 and USP 800 standards. Documents all activities on appropriate logs. Maintains and expands clinical pharmacotherapy knowledge through literature reviews and professional development activities. Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules. Qualifications

Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred Registered Pharmacist License in state(s) of practice Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy Experience with compounding infused drugs Proven record of evaluating and monitoring drug therapies for home care patients Good organizational and time-management skills Participation and membership in professional societies and organizations is desirable Excellent verbal and written communication skills Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis Travel: 0-25%; minimal travel may be required Physical Requirements To perform this role constant sitting and typing on a keyboard with fingers is required, with occasional standing and walking. The physical requirements include the ability to push/pull and lift/carry 10-30 lbs. About Our Line Of Business
