This is a Home Health Fieldwork position doing patient home visits in the Volusia County. The Role You'll Contribute Coordinates and directs the care of a caseload of home patients requiring occupational therapy as the primary skill. Performs an evaluation, assessing function using a method which objectively measures activities of daily living. Assesses physical, functional, psychosocial, social, spiritual, educational, developmental, cultural, cognitive status and discharge planning needs of the home care patient. Utilizing assessment data, formulates a patient specific plan of care along with the patient, family and physician. Bachelor's Degree
or
Master's Degree
in Occupational Therapy, active American Heart Association BLS, and valid in-state Driver's License with current car insurance.
