Overview
The Behavioral Health Specialist provides mental health and substance use assessments, reassessments, services plan, short-term therapy, crisis intervention and case management services to PATH Center patients. Follows-up to ensure continuity of care as the patient uses outpatient, inpatient and community-based services. Participates with the team on care coordination, multidisciplinary team meetings, QI activities, referrals/linkages and trainings. Responsibilities

Master’s Degree in Social Work or Mental Health Counseling Two (2) years of experience managing and performing social work and medical case management in an HIV clinical setting. Experience should include short and long term counseling, crisis management and medical case management. Preferred Experience working with HIV+ families, adolescents, children, and newborns in a hospital or health care type setting. Preferred Counseling experience with adults and adolescents; and substance use counseling New York State Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) Bilingual English / Spanish Broad knowledge of community resources and social work/case management regulatory requirements.
