Nurse Practitioner - Family Physician Job In Connecticut Facility is seeking an experienced Nurse Practitioner or Physician Assistant to join our Adult Medicine Clinic Responsibilities include: Provide comprehensive primary care, health promotion, and disease prevention in collaboration with department physicians. This role includes patient consultations, assessments, diagnosis, treatment, and specialist referrals as needed. Strong clinical skills in taking medical history, assessing conditions, and interpreting findings. Ability to provide culturally competent care to a diverse patient population. Experience with electronic health records (EHR) and medical documentation. 1-2 years' experience in an ambulatory or urgent care setting preferred. Excellent communication skills and ability to work in a collaborative, team-based environment.
