Overview
HealthCare Support is seeking an Advanced Practice Clinician – Nurse Practitioner or Physician Assistant in Albany, NY.
Responsibilities
Travel to two facilities in Albany and Rensselaer counties to perform comprehensive patient assessments addressing medical and behavioral conditions.
Manage acute and chronic illnesses with accurate and thorough documentation, including ICD-10 conditions.
Collaborate with the interdisciplinary care team, conduct advanced illness and care planning conversations, and provide counseling and education to patients and caregivers.
Schedule: Monday through Friday, 8:00 am–5:00 pm.
Qualifications Required Qualifications
Active and unrestricted license in New York
Graduate of an accredited MSN, DNP, or MPA program
Board-certified NP (through ANCC or AANP) or PA
Current, active DEA licensure/prescriptive authority
Full COVID-19 vaccination
Preferred Qualifications
1+ year of clinical experience in practice (long-term care setting preferred)
Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
Understanding of geriatrics and chronic illness
Understanding of advanced illness and end-of-life discussions
Proficient computer skills including EMR documentation
Pay and Benefits
Pay: $85.00–$95.00 per hour
