Clinical Pharmacist, Infusion Center, 40hr, Day at UMass Memorial Health. Exemption Status: Exempt. Schedule Details: Monday through Friday, On Call - Required, Weekends (Saturday and Sunday). The clinical pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research. Responsibilities include improving the patient experience, safety, and achieving positive clinical and financial outcomes. This position reports to an immediate supervisor/manager. Major Responsibilities

The following key performance areas are performed by the clinical pharmacist: Clinical Performance, Operations/Distribution Performance, Financial Performance, Safety and Regulatory Performance, Research and Education Performance. Clinical Performance

Review patient medication history, educate patients on drug therapy, establish desired outcomes, develop/modify therapeutic plans, determine alternatives for achieving outcomes, develop drug monitoring plans, monitor patient outcomes, document medication-related issues in the electronic health record, interpret laboratory data, review patient medication profiles, monitor pharmacokinetics of drug therapy, monitor for potential adverse interactions. Maintain and update a patient profile with demographics, diagnoses, allergies, and current medications. Monitor drug therapy for appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions. Provide drug information to health care professionals and patients. Document clinical interventions and counsel patients on discharge medications as appropriate. Perform clinical review and verification of medication orders; perform medication order entry when needed. Discuss medication order clarifications with the prescriber, document interventions, and communicate medication order changes as needed. Facilitate medication discharge planning and smooth transition to alternate sites of care and access to an ambulatory pharmacy. Operations/Distribution Performance

Ensure timely and accurate dispensing of medications from the pharmacy area. Ensure proper technique and accurate preparation of all pharmaceutical products, including oral, non-sterile and sterile compounding, chemotherapeutic, hazardous, and investigational agents. Be responsible for accurate filling and dispensing of medications from the pharmacy area. Cross-cover in various practice areas within the pharmacy as requested by supervisor. Ensure regulatory compliance of controlled substance distribution and monitoring. Supervise work of pharmacy technicians and provide feedback to maintain/optimize pharmacy technology. Financial Performance

Promote formulary drugs and convert non-formulary orders to formulary when possible; coordinate procurement of non-formulary drugs when necessary. Conduct targeted drug programs and drug usage evaluations and report results to Department Managers/Pharmacy and Therapeutics Committee. Identify cost avoidance in drug therapy by promoting cost-effective regimens and reducing waste; facilitate cost saving programs such as IV to PO and therapeutic substitution. Manage medication inventory, expiring products, and recalls. Safety and Regulatory Performance

Detect, monitor, document, and report adverse drug reactions and medication errors. Use drug use reports to identify adverse drug events. Participate in Joint Commission and other accreditation and regulatory activities such as National Patient Safety Goals and Department of Pharmacy Quality Improvement Plan. Research and Education Performance

Precept IPPE/APPE rotation students and pharmacy residents consistent with credentials and clinical experience. Participate in continuing education programs, meetings, training, and related activities. Prepare educational materials and participate in information sharing for the health care community and general public. License/Certification/Education

Position Qualifications

Required

Doctor of Pharmacy (Pharm.D) degree; OR Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience. Licensure with the Massachusetts Board of Registration in Pharmacy. Preferred
