Overview

The Nurse Practitioner (NP) is responsible for the assessment of selected patients in the designated service. The NP performs physical examinations, confers with patients in person and by phone/electronically, collaborates with others in the department to assure smooth flow of patient care consistent with established quality indicators and participates in the overall coordination of patient care. The NP reports to the Chief of the designated service and to the Department Manager. Responsibilities

Practice under the standard procedure established for the NP role consistent with Kaiser Permanente policy and in accordance with requirements set forth by the California Board of Registered Nursing. Conduct age-appropriate assessments of patients, including history taking and physical examination consistent with department protocol. Perform ongoing assessment/data collection focusing on physiologic, psychosocial, cultural, spiritual and cognitive status; educate patients/families, involve families and advocate for patients. Consult with supervising physician or other clinicians as dictated by patient need and assessment findings. Participate with physicians and other clinicians to ensure effective patient care aligned with quality indicators for the designated service. Review medical records for follow up or ordering of diagnostic studies under established protocols. Establish and maintain communication with other health care providers and departments to promote efficiency and continuity of care. Develop effective working relationships with health care team members, patients and families; acknowledge staff rights and cultural/ethical beliefs. Delegate appropriately and coordinate duties/activities of health care team members; evaluate care effectiveness. Demonstrate knowledge of safety principles and apply them; respond to emergencies according to established protocols and document events. Accept responsibility for own professional practice and engage in staff development; understand legal issues including patient confidentiality and risk management. Participate in unit and department Performance Improvement activities and nursing research as requested; identify and solve problems effectively. Maintain a service philosophy in all patient and team interactions; use computers and telecommunication tools to document, track and convey information. If practicing in a specialty role, fulfill role-specific expectations. Perform other related duties as necessary. Qualifications

Experience

Minimum of 6 months experience as a nurse practitioner. Two (2) years (within the past three (3) years) experience as an NP providing specialized care for patients in an emergency department or urgent care setting. Education

Masters Degree in Nursing (unless already a Kaiser Permanente employee who was grandfathered in prior to January 1, 2008 regulatory change). Completion of a Masters Program in Family Practice NP or Acute Care NP required. If only caring for adult or pediatric populations, then Acute Adult NP or Acute Pediatric NP is required. Licensure & Certification

Meet credentialing requirements established for nurse practitioners within the Kaiser system (Master’s Degree in Nursing and National Certification from one of the following organizations: AANP; ANCC; NCBNS; NCPNP). Must meet requirements to apply for PTAN (Provider Transaction Account Number) at the time of hire and apply within 30 days of hire.
