Overview

Nurse Practitioner or Physician Assistant embedded in a Heart Failure Palliative Clinic in Detroit. Full-time, Monday–Friday, 8:00 AM–5:00 PM with in-person and telemedicine clinics; potential for some inpatient rounding. Provide comprehensive palliative medicine consultations to patients with advanced heart failure in the clinic setting, with potential for inpatient palliative consultations. Perform comprehensive palliative care assessments, manage pain and symptoms, support goals-of-care discussions, and counsel patients and families. Assess physical, psychological, and spiritual concerns; facilitate advance care planning and advance directive completion. Collaborate with the interdisciplinary team and primary service to lead goals-of-care discussions; integrate health counseling for patients and families. Educate other clinicians, healthcare administrators, and patients/families on hospice and palliative care concepts. Document clinical notes regarding patient condition, care provided, symptom changes, and advance care planning/goals-of-care discussions; participate in continuing medical education related to palliative medicine. Develop and ensure adherence to quality standards, process-improvement programs, and applicable clinic practices. Adhere to departmental policies, safety, infection control standards, and organizational mission, vision, and values. Participate in the weekly multidisciplinary VAD/Transplant Selection Committee Meeting. Meet required competencies and credentialing for roles, including potential inpatient rounding. Qualifications

Master's degree in nursing (NP) or graduation from an AMA-approved Physician Assistant program; Licensure as a Physician Assistant; current NP licensure in Michigan or eligible and certified as appropriate. Preferred: Palliative and/or Hospice experience; experience as an APP (NP or PA) with strong emphasis on palliative care. Will consider applicants with extensive nursing experience. Current certification as a Certified Nurse Practitioner and certified as Hospice and Palliative Nurse (CHPN) or Advanced Certified Hospice and Palliative Nurse (ACHPN) from HPNA, or certification obtained within 2 years of hire; or equivalent NCCPA certification for PAs; BLS and ACLS/ATLS/PALS as required by department. Current state RN license (if applicable) and Michigan licensure with continued certification by NCCPA for PA; DEA license required before start date. Experience & Skills

Experience as a Palliative Medicine APP (NP or PA) preferred; consideration given to strong APP candidates or experienced nurses. Ability to work in an embedded clinic with outpatient Advanced Heart Failure team; potential for some inpatient rounds at Detroit hospital. Strong communication, collaboration, and documentation skills; ability to integrate evidence-based practices and consider the cost-benefit of care. Education/Experience Required

Master's degree in nursing or a Bachelor’s degree with completion of an AMA-approved Physician Assistant program. Licensure as a Physician Assistant. Certifications/Licensures Required

Current BLS; ACLS/ATLS/PALS provider status as required by department; DEA license required before start date. Hospice and Palliative certification per above (CHPN or ACHPN) within 2 years of hire or equivalent certification; Michigan RN license where applicable; National certification for PA (NCCPA).
