Overview
Oncology Nurse Navigator - Neuro Oncology role at UT Southwestern Medical Center. This position assists patients, families, and caregivers to overcome healthcare barriers and provides ongoing assessments of needs and referrals to Support Services along the Oncology continuum of care. Uses nursing process and evidence-based knowledge to educate and facilitate informed decision making for patients and families. Responsibilities
Assess barriers to care through the cancer care continuum to address patient, caregiver, or family needs. Serve as point of contact for provider, patient, and caregiver. Act as patient advocate to help navigate the healthcare system. Develop and deliver education programs, resources, and tools to address patient and staff needs. Provide education, resources, and referrals to internal and external resources based on assessed needs. Facilitate shared decision making, ensuring patients, families, and caregivers are fully informed about treatment plans, goals, and outcomes. Promote care planning and help patients articulate questions to discuss with their healthcare providers. Facilitate clear communication among providers, departments, and patients. Coordinate multidisciplinary planning and participate in care conferences as needed. Participate in outreach projects to help close patient care gaps. Perform other duties as assigned. Requirements
Education Associate's Degree in Nursing from an approved/accredited nursing program (Texas BON 217.1). Experience 3 years of oncology nursing experience. Licenses and Certifications RN (Registered Nurse) and BLS (Basic Life Support) per American Heart Association or American Red Cross requirements. Preferred
Bachelor's Degree in Nursing (BSN). OCN (Oncology Certified Nurse) or other relevant certifications (PHHN, CBCN, BMTCN, etc.). Benefits
