We are looking for a compassionate and collaborative Bereavement Coordinator to join our caring team. In this vital role, you will walk alongside families and caregivers during one of the most difficult seasons of their lives—offering emotional support, guidance, and comfort. As an integral part of our interdisciplinary hospice team, you’ll work closely with nurses, social workers, chaplains, and others to develop personalized grief support programs, lead support groups, and ensure ongoing bereavement care is provided with sensitivity and heart. Responsibilities
Primarily responsible for coordinating the assessment of grief following the death of the patient, determining the bereaved’s needs and ensuring timely and appropriate services in response. May provide bereavement individual or group counseling in response to anticipatory grief or bereavement as qualified, or make appropriate referrals for such services. Collaborate with the Interdisciplinary Group (IDG) to develop an appropriate bereavement plan of care. Act as a primary resource regarding bereavement information to the community served. Participate in the Quality Assessment and Performance Improvement (QAPI) program, including the regular evaluation of the bereavement program. Participate in program budget preparation as assigned.
