The ADRC Professional will provide information and assistance to the public related to services and resources for older adults and people with physical or intellectual disabilities. This position is responsible of communicating with County residents at locations such as senior centers, senior meal sites, residential facilities, day programs, as well as other community-based sites. The ADRC Professional will contribute to meeting the required ADRC Outreach activities. Job Duties: Pursue, plan, develop, administer, and evaluate outreach activities to create public awareness of the services and resources available to older adults and adults with disabilities in Milwaukee County. Provide 1:1 and small group interaction to directly connect people to available services. Connect customers to community-based agencies, Options Counselors, Dementia Care Specialists, Benefit Specialists, Adult Protective Services, and parties responsible for the senior meals program, transportation, and health care providers. Ability to offer and administer options counseling, enrollment counseling, and/or disenrollment counseling for clients walking into partnering sites such as: Senior Centers, Meal sites, and other partnering agencies. Complete follow-up contacts to ensure customers’ needs were met. Be knowledgeable of and make use of multiple systems to facilitate customer service: Forward Health, FSIA, CARES, online resource databases, MIDAS. Provide decision-support to customers navigating the complex world of long-term care. Achieve and maintainOptions Counseling Certification and AIRS Certification throughout duration of employment. Research information on public/private providers and programs to provide accurate and current information to customers on resource eligibility, cost, service descriptions, and method to apply/receive services. Maintain current knowledge on various types of Medicaid eligibility including Community Waivers, Institutional, and Medicare Savings Programs. Share information on resources with colleagues. Document outreach activities, referrals, and counseling efforts specific to customer served. Documents accurately in a clear, concise, and objective manner. Documentation includes demographic information of the customer served and the action taken on their behalf. Maintain 100%-time reports in 15-minute increments to receive financial reimbursement from the State of WI. Perform other duties. Accept, complete, and report on a variety of other duties as assigned by supervisor or DHHS management. Supporting the efforts of the department collaboratively with other staff builds teamwork and promotes a good working environment. Minimum Qualifications: Current resident of the State of Wisconsin. Must possess a valid Wisconsin driver license and have access to a reliable insured vehicle; able to maintain throughout employment. Bachelor’s Degree from an accredited college or university in social work or in a closely related field. At least one (1) year of experience working in an Aging and Disabilities Resource Center, providing information and referrals, or experience working with older adults and adults with disabilities. Intermediate experience with all related computer and software applications such as Microsoft Word and Excel. Intermediate experience with ForwardHealth and case management systems. Routine shifts hours. Infrequent overtime, weekend, or shift rotation. Intermittent or cyclical work pressures with occasional exposure to high stress work environments. Note:
Options Counseling Certification and AIRS Certification must be obtained after one year of employment. Knowledge, Skills, and Abilities:
Skills needed for success in this role include customer focus, excellent verbal communication skills, patience, ability to write succinctly, professionalism, critical thinking, strong organizational skills, and time management skills. The ability to work collaboratively with other units and organizations within and throughout the county.Proficientin data collection and analysis. NOTE:
