The PRN Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert providing ethical, evidence-based, safe and compassionate nursing care. The RN Care Coordinator focuses on patient safety and quality of nursing care by identifying and clarifying patient needs, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes. The RN Care Coordinator supports assigned physicians by communicating with the patient/family/caregiver and relaying information to the physician, providing support during clinic sessions, ensuring the patient/family/caregiver understands the plan of care, verifying that orders placed during the clinic session are completed, and notifying the physician of results. Responsibilities and Essential Job Functions Responsible for management of patient flow, triage, and direct patient care. Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager. Manage patient care by developing expected outcomes, defining resource utilization, and working with the provider to set patient goals. Provide nursing support/coordination to optimize patient outcomes and improve quality of life through clinical and educational programs and support services. Support patient self-management of disease. Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits. Manage high-risk patient care, including patients with multiple comorbidities or high readmission risk, including a registry. Develop relationships with patients and assess physical, psychosocial, financial, and environmental health needs. Educate patients and appropriate personnel about health care needs, safety, and benefits. Utilize electronic tools (EMR) to document assessments, clinical findings, plans of treatment and progress toward goals. Observe, record and report patient condition and reactions to drugs; educate patient/family about procedures, medications, nutrition, and health maintenance. Maintain and review patient records and test results; follow up with patients; post test results. Respond to treatment-related inquiries and instruct patient/family regarding medications and treatment instructions. Maintain patient confidentiality and document care in EMR in compliance with practice standards. Perform the professional, clinical, and technical competencies required for the assigned unit or department. Note:

High School Graduate Bachelor Degree in Nursing (For existing employees, BSN required within 5 years) 2 or more years RN experience Preferred Education and Experience
Will consider in lieu of RN experience a TUKHS Ambulatory Clinic LPN with 4 years of TUKHS experience in the same clinic setting Previous medical office experience Required Licensure and Certification
Basic Life Support Heartsaver (BLS) – American Heart Association or obtained within 14 days Licensed Registered Nurse (RN) – Multistate license/compact license; current active license in state of work or proof of application for multistate license Staff hired prior to 2/1/2024 must obtain a multistate RN license by next renewal (by 3/31/2026) Equal Employment Opportunity
