Direct Care Counselors
for our newest
Transitional Housing Service program
located in Concord for
second shift . This
16-bed adult program
is situated at the Philbrook Adult Transitional Housing (PATH) site. The program offers
residential treatment
and
day programs
with a focus on
clinical, vocational services
and
life skills
for adults with mental illness seeking independent living. Many participants secure apartments and pursue college with staff support. Responsibilities:
Supervise daily activities, provide support, guidance, and role modeling. Facilitate improvements in personal responsibility, social skills, medication management, community living skills, and behavior. Assist in group facilitation and transportation to medical appointments. Qualifications:
Bachelor’s degree and/or two years of related experience or training; or an equivalent combination of education and experience with the participant population.
