Camden Case Management

is currently seeking Mentors in San Jose. Qualified candidates will preferably have experience working in the mental health, addiction, and/or coaching fields. Some supervision hours toward licensure may be available in this role.

As a mentor, you will be the primary support for your clients, leading in daily communication, observation, and reporting. You will spend most face-to-face hours with your clients, providing in-depth and timely updates to the team. Mentors report to the case manager, their direct supervisor.

Mentor Primary Responsibilities:

Have weekly face-to-face sessions with clients wherever support is needed.

Monitor clients to ensure safety and support.

Conduct home visits.

Assist clients with medication counts and adherence.

Conduct random toxicology screens (urine samples).

Support clients in practicing skills learned in clinical treatment.

Assist with life skills such as resume building, scheduling, budgeting, organizing, goal setting, job support, and personal accountability.

Engage in recovery activities like exercise, meditation, grocery shopping, attending 12-step meetings, and community development.

Provide phone support and maintain ongoing communication with the case manager.

Document all sessions, interactions, and noteworthy events.

This part-time role offers potential for full-time hours. The position requires independence, strong communication, and organization skills.

Reports to: Case Manager

Schedule: Flexible hours, Monday through Sunday

Desired Skills and Experience:

Familiarity with mental health treatment and services (preferred).

Trauma and addiction-informed skills (preferred).

Encouraged to pursue education in mental health, counseling, addiction, or coaching.

CPR certified.

Valid driver’s license and clean driving record.

Organized, able to prioritize tasks.

Strong problem-solving skills.

Flexible schedule including weekends and holidays.

Experience in mental health, recovery, or coaching (preferred).

Empathetic, professional, and effective communicator in stressful situations.

The pay rate varies from $20.00 to $36.00 per hour, depending on assignments and qualifications. Bonuses, tips, and benefits are not included in this range.
