About The Role
The Inpatient Psych Social Worker at Sinai Hospital acts as a coordinator of psychosocial assessment and discharge planning for the patients/clients receiving inpatient psychiatric services at Hospital, providing leadership and psychosocial support necessary to address the individual's needs.
This is a Part-Time position that will be scheduled for 2 Day shifts during the week M - F with no weekend shifts.*
Key Responsibilities
Assessment & Planning: Conducts initial and ongoing assessments to determine patient needs for care coordination and discharge, then develops a focused discharge plan, especially for high-risk patients. Intervention & Collaboration: Works closely with the clinical team and medical providers to put the discharge plan into action. Continuous Improvement: Stays current with healthcare trends, regulations, and payer requirements related to patient care, discharge planning, and benefits.
Requirements
Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Master's in Social Work required. 3-5 years related experience; Prior experience with Behavioral Health case management/discharge planning strongly preferred Current Maryland LMSW, LCSW, LCSW-C or LCPC required MD Social Work License per level of education. Expectation is to participate in a unit or departmental work groups and meetings whereby initiatives and leadership skills are to be exhibited. May supervise an MSW intern as a field instructor.
