Overview

HWC - Clinical Social Worker NE role at GBMC HealthCare. Location: Serving Hagerstown, MD and Pennsylvania. Responsibilities

Under general supervision, provide supportive services to the patient/family unit of care. Assist in adjustment to illness and grief, addressing personal, financial and environmental difficulties associated with end of life. Conduct psychosocial assessments of hospice patients and family members; identify risk for complicated grief and intervene as appropriate. Develop and coordinate a plan of care with the interdisciplinary team; participate in interdisciplinary group meetings. Provide complex social work services including individual, family and group modalities and other recognized psychosocial therapies to support patients and families in adjustment to illness. Interpret patients’ and families’ behavior related to illness and psychosocial factors; offer suggestions for modifying behavior as needed. Facilitate location of care planning, link with community and financial resources to support caregiving; advocate for patients and families within the medical model. Maintain records of social work and bereavement risk assessments, problems, plans, progress and termination per program policy. Provide education/in-services on psychosocial needs to hospice staff and other agencies; participate in orientation and training of volunteers and provide support to volunteers as appropriate. Qualifications

Education: Master’s degree in Social Work from a CSWE-accredited program. Licensures/Certifications: Licensed in Maryland as LMSW, LCSW, or LCSW-C. Advanced licensure for LMSWs required within 3 years of start date; supervision provided. Experience: One year of social work experience in a health care setting. Skills: Strong interpersonal skills to engage patients and families; analytical skills to assess needs and plan care; time management for case management; ability to read and write English for professional documentation. Pay Range: $26.53 - $43.50 per hour. Final offer based on qualifications, education, experience, and alignment with organizational needs. COVID-19 Vaccination
