NCC’s Extended Care Facility is a Long-Term Care/Sub Acute Facility located in Newark, New Jersey The Director of Social Work assists the resident population to attain or maintain their highest practicable physical, mental and psycho-social well-being. He/She will organize, plan, and direct functions of the Social Services Department in accordance with Federal, State, and Local standards, guidelines and regulations that govern Long Term Care. Responsibilities: Implement written objectives, standards of practice, job descriptions, policy and procedural manual and an organizational plan for social services. Prepare annual budget and allocate resources within budget restrictions. Maintain working relationship with administration through conferences, written memoranda, and email, other methods of exchanging information. Supervise the clinical social workers and ensure accurate performance standards are being met for the position. Monitor documentation related to psycho-social information. Develop and coordinate quality assurance program for social services. Evaluate resident’s social, psychological, financial cultural and family situation and incorporate this information into social service treatment plan. Coordinate discharge planning services for residents when applicable. Experience and Skills:

Bachelor’s Degree in Social Work or related field from an accredited college or university. Certified or licensed by the New Jersey Board of Social Work Examiner. Three (3) years prior experience as a Social Worker with knowledge of casework process, admissions, counseling and social work service. The ability to apply that knowledge to residents/family problems. A compassionate, understanding and professional attitude towards residents’ population and peers. Bi-lingual ( English and Spanish), preferred Must respect the principles of resident’s rights. Must have excellent communication skills and the ability to work as a team player. Demonstrate professionalism and sensitivity when interacting with residents, families and co-workers. Maintain appropriate levels of confidentiality and discretion of all residents care information.
