Market Physician Executive (MPE) - Savannah, GA The Monogram Market Physician Executive (MPE) is a mission-driven physician leader dedicated to improving the well-being, quality of life, and health outcomes for our patients. The MPE will lead our in-home multi-specialty polychronic care model in an assigned market, collaborating with local advanced practice providers (APP), registered nurses (RN), licensed clinical social workers (LCSW), and pharmacists (PharmD). The MPE will leverage Monogram Health’s Multi-Specialty Platform to deliver in-home specialty care, ensuring health equity and health equality through a proven risk-based model. Reporting to the Region President, the MPE contributes to the development and oversight of clinical strategies, policies, programs, processes, and operations that drive improved patient health outcomes. The MPE oversees daily clinical and business operations, including direct patient care, care management services, and pharmacy services. The MPE is expected to review and approve care plans and direct treatment plans for our patients while collaborating with community physicians and facilities. Roles and Responsibilities
Clinical Guidelines Execution – 25% Deliver on Monogram Health’s proprietary evidence-based clinical pathways. Ensure adherence to established clinical guidelines and Monogram Model of Care. Review and approve APP, RN, SW, and PharmD plans of care. Document patient activities, interventions, and tasks appropriately and timely within Salesforce and Athena. Clinical and Operations Performance and Quality Improvement – 25% Accountability for reducing total cost of care and Medical Loss Ratio. Responsible for clinical outcomes, including clinical interventions closure and utilization management. Lead daily high-risk and concurrent review rounds. Supervise front-line clinical and operations team members. Delegate operational responsibilities to Market Manager. Present market performance and outcomes to Executive and Senior Leaders. Patient Care and Treatment – 25% Provide direct and indirect patient care, including diagnosis and treatment. Engage with patients on treatment plans and community provider collaboration. Order labs, referrals, and actions to drive patient outcomes. Population Health Management (PHM) – 20% Provide clinical guidance to Market teams for Population Health Management activities. Collaborate with stakeholders to address utilization trends impacting care and outcomes. Conduct patient reviews to target high utilizers and high-risk patients. Participate in Monogram On-Call activities. Position Requirements
This position will be remote with occasional in-home patient visits and domestic travel. Experience applying evidence-based clinical criteria. Experience in renal care and geriatrics. Strong management and communication skills. Active, unrestricted state medical license in each state within the market. Experience with high-need Medicare Advantage and managed Medicaid populations. Experience with NCQA, HEDIS, and quality improvement. MD or DO degree from an accredited medical school. BC or BE in an ACGME approved specialty. About Monogram Health
