HealthRIGHT 360 gives hope, builds health, and changes lives for people in need by providing comprehensive, integrated, compassionate care that includes primary medical care, mental health services, and substance use disorder treatment. Health RIGHT360 offers residential treatment services designed to serve individuals and families with substance use disorders and co-occurring mental health disorders. Our program helps clients address addiction and related issues, increase skills, manage stressors, connect to supportive services, and build a foundation for lasting recovery. Residential programs run 24/7 and include individual and group therapy, family therapy, individualized case management, treatment planning, drug and alcohol counseling, medication support services, and crisis intervention. 890 is a residential treatment program for men with Substance Use Disorders, located near Alamo Square in San Francisco. It is a 99 bed trauma-informed and dual diagnosis program, staffed by an interdisciplinary clinical team of Substance Use Disorder counselors and therapists. 890 utilizes evidence-based curricula to provide individual and group counseling in a residential treatment setting for some of San Francisco's most vulnerable adults. Our goal is to serve clients in an environment where they feel supported and accepted so they can begin their recovery in a safe and peaceful setting. This is a union role. Responsibilities
Individual Treatment Responsibilities : Maintains a caseload of approximately 20 clients, facilitates weekly individual counseling sessions with each participant. Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals. Proactively links clients to internal and external resources based on their treatment needs and follows up on progress/status. Treatment Setting Responsibilities : Facilitates group counseling sessions, community meetings, and teaches classes on various treatment-related topics. Performs crisis intervention and communicates with the treatment team as unforeseen situations arise. Responds to overdose and other medical and behavioral health emergencies. Documents updates and incidents in the facility log daily. Performs periodic safety checks, property and room searches, and substance use testing to ensure safety and security of the facility. As needed, accompanies clients to offsite appointments related to their treatment. Attends required training and meetings related to the position. Plans and facilitates client celebrations and special events. May work weekends and holidays as needed. Documentation Responsibilities : Collaborates with each caseload client and other resources to develop/maintain treatment plans, transition plans, weekly progress notes, and updates in support of health and recovery needs. Properly documents all individual and group counseling sessions and completes discharge paperwork within 24-72 hours, per agency standards. Maintains accurate records by entering documentation into electronic systems in accordance with HealthRIGHT 360 guidelines to satisfy internal and external evaluating requirements. Meets 50% service delivery expectation. Other : Performs other duties as assigned. Education, Knowledge, Skills and Abilities
Education Drug and Alcohol Certification recognized by DHCS High School diploma or equivalent Knowledge Culturally competent and able to work with a diverse population Strong proficiency with Microsoft Office applications, specifically Word, Outlook and internet applications Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency Must complete background check and Livescan clearance.
