Optima Medical is an Arizona-based medical group consisting of 30 locations and over 130+ medical providers, who care for more than 200,000 patients statewide. Our mission is to improve the quality of life throughout Arizona by helping communities “Live Better, Live Longer” through personalized healthcare, with a focus on preventing the nation’s top leading causes of death. We go beyond primary care with a full spectrum of services including cardiovascular health services, behavioral health, allergy testing and immunotherapy, in-house lab testing, imaging, chronic disease management and other specialty health services. Optima Medical is currently seeking a Psychiatric Nurse Practitioner to provide primary care to psychiatry and at-risk patients through virtual communication while working remotely.
Must reside in Arizona or open to relocating to Arizona to be considered for the role
Collaborative Care Model (CoCM) CoCM is a team-based approach to mental health that involves collaboration between primary care providers, behavioral health specialists, and psychiatric providers to deliver integrated and comprehensive care to individuals with mental health conditions. The goal of CoCM is to improve access to mental health services through community-based resources, enhance patient outcomes, and provide more holistic and coordinated care.
Job Responsibilities
Providing Psychiatric support and consultations as a member of our Behavioral Health Collaborative Care Team
Virtually consults with patients to address Behavioral Health concerns
Utilizes quality improvement data to enhance patient care
Weekly meetings with the CMO and Behavioral Health Care Manager Lead
Work directly with care managers and referring PCPs
Job Qualifications
Bachelor’s degree in nursing followed by a Master’s degree in Psychiatric or Mental Health Nursing from an accredited school for professional nursing training
Possess strong understanding of societies and their impact on mental health; ability to solve problems, monitor and examine patients
Be compassionate, sensitive, and possess strong reasoning and thinking skills
Excellent written and verbal communication skills
At least 1 year of clinical work experience as a Psychiatric Nurse Practitioner
At least 1 year of EMR experience; eClinicalWorks preferred
