Develop and implement individualized treatment plans for patients Conduct home visits and provide hands-on therapy Educate patients and their families on exercises and techniques Collaborate with other healthcare professionals to ensure comprehensive care Document patient progress and communicate with the healthcare team Qualifications
Valid Physical Therapist license in California Minimum of 1 year of experience in home health or a similar setting Strong communication and interpersonal skills Ability to work independently and manage time effectively Passion for providing high-quality, patient-centered care About Us
