Position Objective:
Provides psychosocial assessment, individual, family, and group therapy to a caseload of patients. Assesses, plans for, and implements identified services for patients. Works with the patient and family/significant others on a safe discharge plan. Collaborates with the provider and interdisciplinary team on the care plan.

Provides comprehensive psychosocial assessments for patients, creating a therapeutic framework for effective problem solving with patients and families, and responds to mental health needs.
Collaborates with Care Coordinator to provide transition of care strategies in complex situations such as substance abuse, psychiatric, or dual diagnosis placements/services, ventilator-dependent patients, inpatient dialysis, uninsured patients needing skilled nursing placement, patients requiring guardianship, or long-term/custodial placements.
Identifies community services and resources available to patients and families, facilitating linkage with those services.
Collaborates with the interdisciplinary team, communicates patient progress, makes recommendations, and revises plans based on ongoing evaluation. Participates in rounds to address issues and facilitate safe care transitions.
Integrates medical and nursing information into assessments and interventions.
Ensures timely and appropriate evaluation and reporting of suspected child/elder abuse or neglect. Assesses for domestic violence and involves the domestic violence program coordinator as needed.
Documents interventions in the patient's medical record following department standards.
Participates in process improvement activities within the Mental Health and Substance Use Quality Council, identifying opportunities for improvement and making recommendations.

Provides comprehensive psychosocial assessments for patients identified through distress screening, care team referral, or self-referral.
Creates a therapeutic framework for problem solving related to adjustment to illness, disability, critical care, discharge, long-term care, end of life, grief, trauma, abuse, neglect, substance abuse, and mental health issues.
Promotes social work and psychosocial services to the care team, proactively identifying and providing services to patients who may benefit.
Links patients and families with community services and resources.
Collaborates with referral sources, communicates progress, and revises plans as needed.
Evaluates progress toward goals, arranges care conferences, and integrates medical and nursing information.
Ensures proper evaluation and reporting of abuse and neglect, assesses domestic violence, documents interventions, and facilitates support groups and mentoring.
Educational/Experience Requirements:
Master's of Social Work degree (MSW).
Minimum of 1 year of experience in a mental health or substance use program.
Master's degree in Social Work.
Two years of oncology social work experience.
Two years of clinical social work in a healthcare environment.
Required License/Certifications:
Current license as a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker – Certified (LCSW-C).

Pay Range
$35 — $50 USD
