supports individuals in accessing and maintaining housing. We aim to provide Milwaukee County residents with a safety net, decent and affordable housing, and connections to opportunities that improve their quality of life. We utilize available funding sources efficiently and equitably. Job Summary

Under the direction of the Program Manager, the

Community Intervention Specialist (My Home)

maintains a caseload of homeless and formerly homeless participants with targeted disabilities such as severe and persistent mental illness and substance use disorders. This role involves ensuring housing stability, processing annual paperwork, inputting data into Yardi and HMIS databases, conducting Housing Quality Standards (HQS) inspections, and acting as a liaison between case management, landlords, and tenants. The specialist will also advocate for program participants. Job Duties

Manage a caseload of homeless individuals with severe mental illness and substance use disorders. Serve as a liaison between case management, participants, and landlords. Advocate and intervene to maintain housing stability. Provide direct case management services to participants and families experiencing domestic violence and/or sex trafficking. Input and verify participant information in databases. Conduct HQS inspections of units. Determine eligibility and certify program participants. Attend relevant trainings and meetings. Travel within the community up to 70% of the time. Perform other duties as assigned. Qualifications Current resident of Wisconsin at the time of application. Valid Wisconsin Driver's License and access to a reliable insured vehicle, maintained throughout employment. Ability to travel extensively within the community. Bachelor’s degree or higher in Social Work or a related Human Services field from an accredited institution. At least one year of experience working with the homelessness population or in supportive housing. Intermediate proficiency with Microsoft Word and Excel. Knowledge, Skills, and Abilities Knowledge of housing programs, resources, and the supportive housing model. Ability to work with individuals facing eviction or housing instability and manage county funds for qualifying individuals. Note:
