What you’ll do as a Home Health Speech Therapist/Language Pathologist (ST) In partnership with the Case Manager, you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. Current unrestricted CA Speech Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care Knowledge of state and federal homecare regulatory guidelines preferred Experience in home health care is highly preferred Experience with high acuity patients is a plus Experience working with an interdisciplinary team is highly desired Case Management fundamentals are highly desired Self-Directed; Empathetic; Critical thinker; Outstanding interpersonal skills; Leadership skills; Organized Before scheduling an in-person interview, candidates are required to complete a computer literacy assessment to ensure capability with electronic charting and web-based communication applications. The assessment results are confidential and used as part of the hiring process $110,000 - $125,000 a year Anticipated wage range for Speech Therapist: 110,000.00 - 125,000.00. Starting pay is commensurate with relevant experience. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (up to 4% match). Note: You will be assigned a primary location; however, you may be asked to provide coverage outside of that area to meet client care needs.
