This is an entry level clinical pharmacist position. The Clinical Pharmacist I-Retail supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates competency in all basic clinical pharmacist skills. 1.Customer Service Qualitya. Demonstrate and promote CHAMP behaviorsb. Provide excellent customer service to patients and other healthcare team members, with attention to the needs of all staff, with positive feedback from staff and co-workersc. Provide medication counseling to patients and families, as applicable to the work aread. Demonstrate effective oral and written communication skills e. Demonstrate ability to escalate issues to management and/or collaborating prescribers as appropriate 2.Medication Managementa. Conduct timely, accurate medication order review that considers all patient characteristics including lab values and applicable vital signsb. Provide medication therapy monitoring for assigned patients, adjusting medication therapy based on patient factorsc. Develop an effective interdisciplinary relationship with the health care teamd. Function as a source of medication information for the health care teame. Participate in patient-centered initiativesf. Ensure clinical continuity where applicable to the work area 3.Medication Preparation and Distributiona. Demonstrate competency in pharmacy automation and technology as applicable to the work areab. Oversee non-sterile compounding and medication re-packaging to ensure compliance with regulatory statutes (e.g. USP Chapters), as applicable to the work areac. Oversee medication distribution/dispensing process in the work area, to ensure safe, effective, timely medication distribution and prescription dispensing 4.Medication Safety, Regulatory Affairs, and Quality Assurancea. Adhere to all departmental policies, procedures, protocols, and guidelinesb. Adhere to appropriate state and federal regulationsc. Report medication events and near missesd. Demonstrate HRO principles and conduct in all interactions with patients and staffe. Participate in quality assurance activities 5.Leadershipa. Provide education and training to other pharmacy staff members, pharmacy students and residents, and health care team membersb. Participate in professional development activitiesc. Attend departmental staff meetingsd. Oversee pharmacy staff as necessary in the work area 6.Financial/Volume Resultsa. Contribute to departmental cost-reduction/revenue generationb. Participate in pharmacist-driven cost management protocols where applicablec. Provide cost-effective clinical care Qualifications

EDUCATION Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree EXPERIENCE Experience as a pharmacist in a retail and/or hospital environment is preferred. Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. LICENSURE CT Pharmacist Licensure required, active, and without restrictions. If required, will be responsible for obtaining out-of-state licenses to register for non-resident pharmacy licenses. BLS (Basic Life Support) certification and immunization certification required. MTM (Medication Therapy Management) certification preferred SPECIAL SKILLS This individual should demonstrate basic knowledge of pharmacology and therapeutics and understand pharmacy systems of drug distribution and formulary management.
