PHYSICAL THERAPIST (PT)
St. James Rehabilitation & Healthcare Center is seeking an experienced PT for our facility.
Physical Therapist (PT) Duties Include: The Physical Therapist is responsible for assessing referred patients requiring rehabilitation services. This includes providing direct patient care to evaluate their medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation. The therapist will establish and administer treatment programs with specific goals based on the patient's capacity and tolerance, under the physician's guidance. Additionally, the therapist must set realistic and achievable goals, document patient conditions, and educate patients and families in appropriate physical therapy methods.
Requirements/Qualifications:
Graduated from an accredited Physical Therapy college or university.
Valid professional license and registration for the state of practice.
Responsible for overseeing and supervising Physical Therapy Assistants.
Experience with long-term care and/or geriatric care is preferred but not required.
Proficient in documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.
Collaborates with all disciplines to provide patient-centered treatment and care.
Exceptional patient service skills and clinical excellence.
Punctuality, professionalism, and adherence to scheduled hours.
Performs duties as assigned by the Director of Rehabilitation.
Maintains confidentiality and complies with HIPAA and Federal Residents' Rights.
Experience with Case Mix Index and PPS scheduling is a plus but not mandatory.
Full vaccination against Covid-19.
