Schedule: Full-time (0.9 FTE) | Day & Night Shifts Available
Sign-on Bonus: $10,000 for eligible external hires
Pay Range: $45.67 – $85.73 per hour

Key Responsibilities
Coordinate care and discharge planning for patients in an acute inpatient setting
Perform comprehensive assessments and develop individualized care plans
Collaborate with physicians, nurses, and interdisciplinary teams to ensure continuity of care
Support appropriate resource utilization and advocate for patients and families
Qualifications
Associate Degree in Nursing required; BSN preferred (or completed within 3 years of hire)
Current RN license in the state of Washington
Minimum 2 years of acute care experience in a med-surg or inpatient setting
Prior experience in care management or utilization review preferred; case management certification a plus
