Position Summary:
Join NeighborHealth's PACE program as a Social Worker, engaging with a dynamic Interdisciplinary Team to develop care plans for participants wanting to live safely in their communities. Work involves psychosocial assessments, counseling, group activities, and care coordination, with a focus on adults 55+ with experience with older adults preferred. Must have an MSW and be licensure eligible as an LCSW or LICSW in MA. Bilingual Spanish skills are a plus.
**Responsibilities include:** - Conducting psychosocial assessments - Developing and implementing care plans - Providing caregiver support and leading family meetings - Participating in team assessments and care planning - Leading clinical activities such as group counseling and end-of-life care - Managing admissions, discharges, and psychiatric care coordination
