Position Overview: The Intake Coordinator serves as the first contact for incoming patient referrals and plays a key role in coordinating care for new clients. Responsibilities include managing multiple referrals, ensuring accurate documentation, and collaborating with staffing to match clients with qualified caregivers. Key Responsibilities: Manage daily incoming patient referrals from hospitals, case managers, insurance providers, and families. Maintain a referral pipeline and track each case through intake stages using internal systems. Communicate effectively with patients, families, and referral sources to gather documentation and care requirements. Assess patient needs to match them with suitable caregivers based on skills, availability, and preferences. Collaborate with staffing and HR to ensure timely service initiation. Follow up on pending referrals and ensure completion of intake documents. Stay updated on service offerings, payer requirements, and regulatory guidelines. Support smooth transition of new patients from intake to active service. Ensure timely staffing and movement within the pipeline. Qualifications: Experience in healthcare intake, scheduling, or coordination (home care experience preferred). Strong organizational and multitasking skills—able to manage multiple cases.
