PACE Place provides coordinated medical and social services to elders who want to remain in their home but need support to do so. PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care.

Position Summary The PACE Certified Nursing Assistant (CNA) provides personal care and support services to participants. Responsibilities include assisting participants with activities of daily living, light housekeeping, emotional support, and transportation escort services as outlined in each participant's care plan. The CNA ensures participant comfort, safety, hygiene, and dignity while maintaining compliance with policies and care standards. Services are provided in participant homes or other care settings as scheduled.

Position: PRN as needed. M-F (Days) Essential Functions

Assist participants with personal care and activities of daily living (e.g., bathing, dressing, grooming). Provide light housekeeping duties such as laundry and cleaning as needed. Offer emotional support and companionship to participants and their families. Observe and report physical, emotional, or safety-related changes. Record vital signs such as pulse, respiration, weight, and temperature when necessary. Support restroom activities and maintain cleanliness of participant restrooms and shared spaces. Guide and participate in recreational activities, and assist with meal/snack service. Maintain accurate and timely documentation in the EMR system. Uphold participant confidentiality and comply with HIPAA guidelines. Adhere to infection control practices and biohazard precautions. Attend staff meetings and required training programs. Participate in quality assurance and performance improvement (QAPI) activities. Follow all organizational policies, procedures, and applicable safety regulations. Engage in emergency response and disaster preparedness activities. Perform additional duties as assigned. Qualifications

Knowledge and Critical Skills

Understanding of CNA principles, standards, and practices. Familiarity with safety precautions in home care settings. Basic knowledge of medical equipment operation and maintenance. Strong interpersonal and communication skills. Ability to stay calm and act appropriately during emergencies. Basic computer literacy and documentation skills. Experience

Minimum of 6 months' experience providing care in home settings preferred. At least 1 year of experience working with frail or elderly populations preferred. If under 1 year of experience, must complete specialized training on elderly care provided by the organization. Must meet established competencies before working independently. License / Certification

Current Florida CNA certification required. Valid Florida driver's license and active automobile insurance required. CPR with First Aid or BLS certification required. Level II AHCA background check required. Education

High school diploma or GED equivalent preferred. Other Requirements

Medical clearance for communicable diseases and up-to-date immunizations required prior to direct participant contact. Must demonstrate position-specific competencies prior to assuming care responsibilities. Only act within the scope of his or her authority to practice. Physical Demands and Working Conditions

Must be able to lift, transfer, reposition, and assist participants with mobility. Work is performed in participant homes and other care environments.
