Overview Provides speech-language pathology services which include evaluation, management, and treatment to patients referred by physician departments. Provides services in a manner that is consistent with the department's mission statement and responsive to the unique needs of customers including favorable clinical outcomes, quality of service and access, and fiscal accountability.

Responsibilities

Evaluates patients, families and home environment on admission to home health and during home visits. Determines if home environment is safe and appropriate for provision of home health services. Interprets data about patient status. Assesses patients' physical, functional and psychosocial status. Identifies problems accurately. Establishes measurable goals. Develops or revises an appropriate plan of care. Communicates and coordinates plan of care with physician, patient, caregivers, and other health care providers. Determines whether other services (nursing, PT, OT, HCA or MSW) are indicated.

Provides speech-language pathology services as prescribed by patient’s physician and in compliance with Kaiser Home Health Agency standards, policies and procedures. Evaluates patient response to interventions.

Recognizes and interprets significant physiological and behavioral changes in patient condition.

Modifies/updates/changes plan of care when indicated by patient condition and/or physician orders.

Determines in conjunction with physician when patient care should be shifted to another setting (e.g., hospital, clinic, emergency department, out-patient rehab) and/or patient denial of home health, if warranted.

Provides case management for assigned caseload. Coordinates all necessary services to be provided to patients. Integrates all services into the plan of care.

Documents according to Home Health standards and Federal and State regulations. Maintains medication profile for all medications the patient is taking. Documents communication in care planning/coordination. Ensures the medical record is accurate, legible and complete. Completes accurate daily visit log. Submits documentation within required timelines.

Supervises care provided by Home Care Aide (HCA). Reevaluates patient per Home Health standards; conducts and documents supervisory visits.

Maintains infection control procedures and a clean and safe environment for patient care supplies and equipment.

Demonstrates professional growth through participation in conferences, audits, meetings, in-services, continuing education, and teaching activities.

Exhibits effective listening and communication skills. Functions as a team member and role model for other employees. Conducts oneself in a service-oriented manner with patients, visitors, and colleagues.

Utilizes critical thinking including problem solving, reflective practice, clinical reasoning, and prioritizing.

Participates in performance improvement and supports the Quality Improvement process. Assists with development and implementation of department work plans.

Maintains productivity levels per home health standards.

Prepares weekly schedule of patient visits and informs Intake Coordinator of schedule changes and openings for admissions or additional visits.

Follows all applicable laws, rules and recommendations for safe driving when on duty.

Applies knowledge, skills, and abilities necessary to provide care appropriate to populations served.
