Overview
Physical Therapist (PT) - Full-Time at The Phoenix Rehabilitation and Nursing Center, part of the CareRite Centers Network. The Phoenix is a 400-bed facility with a state-of-the-art Rehabilitation Gym servicing 60+ short-term beds. The Rehabilitation team consists of 30+ Therapists with backgrounds in acute, sub-acute, long-term and outpatient settings. We are a 7-day-a-week rehabilitation facility. In this role, you will have the opportunity to work with a diverse patient population on various sub-acute care services including post-surgical, orthopedic, neurology, post-stroke, cardiac, and much more. Responsibilities
Responsibilities Include for Physical Therapist: Assess referred patients requiring rehabilitation services and provide direct patient care to evaluate medical condition, functional capabilities, limitations, and rehabilitation potential. Establish and administer a treatment program with specific goals determined by the patient’s capacity and tolerance under the direction of the Physician. Set realistic and achievable goals, document and record the patient’s condition, and educate patients and families in appropriate physical therapy methods. Qualifications/Requirements
Must have graduated from an accredited Physical Therapy program. Must have a valid professional license and registration for the state of NY. Responsible for overseeing and supervising Physical Therapy Assistants. Experience with long-term care and/or geriatric care is preferred but not required. Well-versed with documentation writing; evaluations, weekly progress notes, recertification and discharge documentation. Collaborates with all disciplines to provide patient-centered treatment and care. Exceptional patient service skills and ability to demonstrate clinical excellence. Report to work on time, adhere to scheduled hours, and demonstrate professionalism at all times. Perform other duties as assigned by the Director of Rehabilitation.
