Reporting to the Deputy Director, the Program Coordinator | Adm is a key member of the IRA/ICF management team, helping to ensure quality care, appropriate placements and overall compliance with state, federal, and CARF regulations and standards. The Program Coordinator is responsible for working with team members and staff to keep the IRAs/ICF in full compliance with regulations and guidelines and audit ready. They are the primary leader in coordinating and overseeing resident transitions and placements, including admissions, discharges and returns from hospital, rehab or nursing home stays, as well as communication to engage and coordinate care with family members. They actively partner and collaborate with colleagues and staff, as well as external stakeholders, including families, advocates with OPWDD and DOH. Key Responsibilities
Responsible lead, in partnership with IRA/ICF management team, for keeping the IRAs/ICF audit ready and in compliance with state, federal and CARF regulations, guidelines and standards of care. Knowledgeable of state, federal and CARF regulations and standards of care, working with Compliance and other sources to build understanding and to keep up-to-date with revisions. Develops, maintains, reviews and reports out on weekly data and trackers. Conducts chart and documentation reviews. Participates in the development of Plans of Corrective Action (POCA) in response to external audits by OPWDD, DOH or other regulatory bodies, and executes and monitors implementation of the POCA. Helps develop and implement internal corrective actions in response to internal findings, observations and audits. Assists in preparation for and facilitation of external audits and CARF accreditation. Tracks and ensures completion of required training for IRA/ICF staff, in collaboration with IRA/ICF management team members. Tracks incidents, ensuring timely follow up on incident recommendations and closure. Primary leader coordinating and overseeing resident care, transitions and placements, including admissions, discharges and returns from hospital, rehab or nursing home stays. Co-facilitates Admission/Discharge Committee and tracks admission, discharges, and related stays, participating in related meetings. Coordinates plans for appropriate placement based on individual need and setting internally with medical, nursing, clinical and residential teams and externally with hospitals/rehab/nursing homes, families, advocates and OPWDD. Facilitates meetings and communication, as required. Documents contacts and plans. Escalates challenging resident care and placement situations up for division and agency support. Facilitates and tracks all required steps for admission and discharges to the IRAs/ICF. Key Member of IRA/ICF Management Team
Coordinates care across disciplines and programs in a respectful, effective and collaborative manner. Assists with and responds to emergency situations. Provides coverage for vacant positions or for colleagues PTO, on a temporary basis. Implements and supports system changes. Utilizes data and proven performance improvement tools, such as Lean Six Sigma, to identify and solve problems. Participates, with the IRA/ICF management team, in meetings, communication, planning and operations. Travels to IRAs/ICF, as needed. Other duties as assigned. Core Competencies

Education and Experience
Educational: Master’s degree in a relevant field required, OR advanced certification in healthcare management, administration or quality improvement. Experience: A minimum of 5 years of experience working with the intellectually disabled population; a minimum of 2 years in a supervisory, compliance or quality role; knowledge of OPWDD and DOH regulations; Hebrew, Yiddish or Russian-speaking preferred. Skills
Computer skills: Strong computer skills; proficiency with Microsoft Word, Excel, PowerPoint; familiarity with electronic health records. Dexterity: Visual and manual dexterity to type reports, review charts, and participate in calls or video conferences. Work Environment & Physical Requirements
Ability to visit multiple homes daily; navigate stairs; interact with residents. Respond to crises or emergencies; move as needed (walking, sitting, standing, bending).
