A community-based outpatient mental health program is seeking a Substance Abuse Specialist to join its Assertive Community Treatment team. This is a clinical role providing recovery-focused, harm-reduction services to individuals with co-occurring mental health and substance use conditions.

Monday to Friday Hours:
35 hours/week Program:
Assertive Community Treatment (ACT) Responsibilities: Manage a caseload assigned by the Team Leader Conduct community-based individual and group counseling with a focus on harm reduction and recovery Complete all assigned field visits within the standard 7-hour service day Transport clients when needed (e.g., from hospitals or court appointments) Facilitate substance use-focused group sessions tailored to client treatment plans Complete initial and ongoing substance abuse assessments; adjust interventions accordingly Clinical Documentation & Assessments for Treatment plans, risk assessments, wellness/safety plans Submit progress notes on the day of service Complete discharge planning and conduct 30, 60, and 90-day follow-ups post-discharge Assist with administrative and team support Coordinate with medical staff to ensure timely documentation post-hospital discharge Maintain a real-time Outlook calendar with daily visits and mileage Report any client incidents or critical events in line with agency protocol Perform other duties, as needed Qualifications: 1+ year of experience in a Clinical and/or Rehabilitation role/setting Master's Degree in Social Work, Mental Health Counseling, Psychology, or Rehabilitation Counseling At least 1 year of clinical or rehabilitation experience Solid assessment and documentation skills Experience with Mental Health illness affected populations and/or Substance Use Disorder individuals
