Position Summary

The

Antibiotic Stewardship Clinical Pharmacist

will serve as a unit-based pharmacist, verifying orders and providing specialized clinical services in antibiotic stewardship and infectious disease. This role will be a critical drug information resource for nurses, physicians, patients, and families.

Responsibilities

Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals. Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions. Deliver medication therapy management with a focus on

antibiotic stewardship and infectious disease protocols . Ensure pharmaceutical care programs are appropriately integrated throughout the institution. Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to antimicrobial therapy management. Conduct Medication Use Evaluations (MUEs) with a focus on antibiotics/ID, and recommend necessary changes to pharmacotherapy practice. Serve as a resource in antibiotic stewardship initiatives, ensuring best practices and compliance with institutional standards.

Qualifications

Education : Required: PharmD or BS in Pharmacy Experience : Required: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years) Strongly Preferred: Infectious Disease training and/or experience in

antibiotic stewardship Certifications : Required: Current Pharmacy license in Massachusetts Preferred: Board Certification in Pharmacotherapy (BCPS) or actively working toward certification Note: Tenet compliance statement regarding vaccination and accommodations remains as provided in the original description.
