Medical Records Director role at PACIFIC VILLAS POST ACUTE. This position is responsible for directing the medical records/health information management functions in accordance with established policies and procedures. Responsibilities

Receive and follow work schedule/instructions from supervisor as outlined in policies and procedures. Organize, plan, and direct the medical records department in accordance with established policies and procedures. Assist the Medical Records/Health Information Consultant as required; maintain minutes of meetings; file as necessary. Develop and maintain good working rapport with inter-department personnel and other departments to ensure proper maintenance of medical records. Record all incidents/accidents and file in accordance with policies; retrieve resident records (manually/electronically); deliver as necessary. File information such as nurses\' notes, resident assessments, progress notes, laboratory reports, x-ray results, and correspondence into resident charts; collect, assemble, check, and file resident charts as required. Assist MDS Coordinator in scheduling assessments in accordance with OBRA guidelines; ensure incomplete records/charts are returned for correction. Develop procedures to ensure resident records are properly completed, assembled, coded, signed, indexed, and filed; establish controls to prevent charts/records from leaving the medical records room without authorization. Maintain records of authorized information released from charts/records, including recipient, date, department, etc.; abstract information for insurance companies, Medicare, Medicaid, VA, and other entities in accordance with privacy rules. Index medical records as directed; maintain registries (e.g., admission and discharge); transcribe and type reports for physicians as necessary; collect and inspect charts for completion. Pick up and deliver resident medical records from wards, nurses\' stations, and other areas as needed; batch resident information into the computer and retrieve demographic information as instructed. Answer telephone inquiries concerning medical records functions; prepare written correspondence as necessary; retrieve records when requested by authorized personnel; ensure records are signed out and signed in upon return. File active and inactive records according to policy; maintain confidentiality and report any violations related to accessing resident information to the Administrator. Agree not to disclose assigned user ID/password and promptly report suspected violations. Committee, Personnel and Other Functions

Perform secretarial duties for facility committees; collect/assemble records for committee review and prepare reports as directed. Report known or suspected incidents of fraud to the Administrator; ensure computer workstations are logged off and secure when unattended; supervise staff and enforce discipline as needed. Maintain census/status changes; participate in mandatory facility in-service training (e.g., OSHA, HIPAA, etc.); attend approved workshops and seminars. Report unsafe conditions or equipment malfunctions; ensure supplies are replenished and work areas are clean and secure. Qualifications

Education and/or Experience: Must possess at least a high school diploma or GED. Must type a minimum of 45 words per minute and be proficient with dictation equipment. Working knowledge of medical terminology, anatomy and physiology, legal aspects of health information, coding, indexing, etc., preferred but not required. On-the-job training provided in medical records and health information system procedures. Language Skills: Must be able to read, write, speak, and understand English; able to read technical procedures. Mathematical Skills: Ability to apply fractions, percentages, ratios and proportions to practical situations. Reasoning Ability: Able to make independent decisions when warranted; able to work with staff, residents, and visitors; willingness to incorporate new methods and principles; follow written and oral instructions; not pose a threat to health or safety. Physical Demands: Must move intermittently; able to lift up to 25 pounds; able to meet health requirements; able to work with distractions and emergencies; may assist in evacuations. Work Environment: Office and facility settings; may require extended hours, weekends, holidays; possible exposure to hazards, infectious materials, and noise; require interaction with residents and staff and occasional exposure to hazardous chemicals.
