The Care Partner is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in the Terrace Club neighborhood based on the Designated Care Partner concept. This role promotes alliance with residents to help create successful days through prompting, cueing, and reminders. It involves providing hands-on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe, home-like environment. The role demonstrates the Sunrise Mission, "to champion quality of life for all seniors" in accordance with applicable standards, regulations, and Sunrise policies. Responsibilities
As part of the Sunrise team, support the Mission, Principles of Service, and Core Values. Work with residents to promote successful days through prompting and cueing. Document and review daily logs, ISP inputs, and Leisure Assessments. Assist in the Terrace Club Program according to resident interests and Terrace Club guidelines. Support daily program calendars and journal entries with residents. Use written communication effectively to assist residents with daily journals. Observe, report, and document symptoms and conditions; monitor health changes. Maintain designated group accountability during shifts and check on residents regularly. Attend required meetings and inform the supervisor if a resident’s care needs increase. Monitor skin condition, medication side effects, and overall health changes; communicate concerns to supervisor and families as appropriate. Respond to security systems, answer call bells promptly, and take appropriate action. Answer and direct phone calls professionally; assist with personal care as indicated (e.g., toilet assistance) and ensure sanitation. Communicate with families and act as a Designated Care Partner resource. Maintain resident dignity, safety, and rights; foster a warm, calm environment. Follow established safety regulations and universal care precautions. Risk Management and General Safety
Ensure compliance with health and safety regulations and promote risk management policies. Practice safety procedures, including PPE, fire safety, MSDS, and Lockout Tagout as applicable. Report accidents/incidents and unsafe conditions promptly. Keep hazards out of walkways and exits; comply with fire codes. Follow infection control procedures and safe disposal of hazardous materials. Know how to respond to blood-borne pathogens and hazardous materials safely; report exposures promptly. Ensure oxygen tanks are stored safely and monitored for prescribed levels. Housekeeping & Laundry Services
Maintain and clean resident rooms, including making beds, trash removal, organization, and cleaning as needed. Wash and manage resident laundry per ISP and encourage resident participation. Dispose of incontinent products safely using hygiene precautions. Maintain dining room linens and complete housekeeping tasks; keep common areas clean and tidy. Dining Services
Serve meals in the dining room and assist with dining room operations as assigned. Promote a pleasant dining experience; follow resident dietary needs as indicated in the ISP. Assist with meal setup/clean-up and participate in pre-meal meetings. Observe and report changes in residents’ eating habits; provide room service as needed. Maintain Sunrise hospitality and service standards with residents and guests. Training and Team Contribution
Participate as a team member and contribute to team goals; uphold our Credo and Principles of Service. Engage in engagement programs and surveys; attend regular department and team meetings as directed. Maintain required training compliance; perform other duties as assigned. Core Competencies

To perform this job successfully, individuals must be able to perform each essential duty satisfactorily. Requirements listed reflect the knowledge, skills, and abilities required. High School diploma / GED; may be required per state/provincial regulations Must be at least 18 years of age Previous experience with seniors preferred and a desire to care for seniors Ability to make decisions in residents’ best interest and respond calmly in difficult situations Effective written and verbal communication; good organizational and time management skills Demonstrates sound judgment, problem-solving, and decision-making abilities If applicable, Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and understand safety duties New Jersey: Possess CNA or HHA certification for new hires after 03/14/04 About Sunrise
