Senior Health Information Management Specialist – (250002V0)

Overview The Senior Health Information Management Specialist will perform quality improvement monitoring necessary for organizing and maintaining an electronic patient record system. The role will coordinate and prioritize daily operations of the electronic health record section and cross‑train in all core Health Information Management functions.

Responsibilities

Act as a resource for projects for medical records systems.

Assist with the barcoding of new and revised medical record forms.

Actively participate in orientation and training of all HIM Specialists.

Retrieve patient identification necessary to appropriately identify patients 100% of the time.

Utilize the appropriate queues in the document imaging system to process work.

Able to use multiple health information management computer applications with a high degree of competency.

Spend 2.5 hrs daily reviewing patient records scanned and indexed with a representative sample of all specialist work (including new employees).

Correct errors and provide feedback to HIM Specialist, HIM Manager and Director.

Prepare quarterly statistics of quality findings to HIM Manager and Director.

Prepare examples of common errors and participate in a group discussion as an individual effort.

Meet with employees monthly along with manager to provide feedback.

Ensure scanned paperwork is not destroyed until the defined sample size is quality assured.

Prepare all documents for scanning into the document imaging system at a rate of 3.0–3.5 in/h following standardized prepping procedures.

Remove all records that are not able to be scanned and initiate a multimedia folder.

Mark all documents which are not of good visual quality.

Remove all COLD documents and do not scan.

Label all batches with appropriate clinic information, date received and processed.

Identify all documents with patient’s name and medical record number.

Scan all documents into imaging system at a rate of 35 in/7‑hr shift following standardized scanning methods.

Use sound judgment when scanner settings need to be adjusted to increase efficiency and quality of the documentation.

Perform daily scanner maintenance by cleaning rollers and glass to prevent buildup of paper residue, calibrate scanner and report any issues.

Avoid having excessive carry‑over of work and communicate with HIM Specialist when daily work assignment is not completed.

Electronic indexes scanned images from their corresponding queues within the document imaging system at a rate of 14 in/shift for quality control and validation is 12 in/7‑hr shift for outpatient and inpatient records.

Maintain accuracy rate at 95%.

Maintain order of all batches of charts/documents scanned and awaiting scanning according to standardized procedure 100% of the time to ensure retrievability.

Follow standard procedure for chart storage and destruction policy.

Utilize electronic patient record system to locate individual patient records and sets of patient records on an as‑needed basis by the setup and retrieval of workflow assignments within 24 hr of routine request and immediately for STA T requests.

Process Death Certificates in accordance with hospital policy and procedure.

Participate in on‑call rotation for death certificates.

Participate in phone coverage rotation within the department.

Process requests for records from outside providers for continuation of care purposes.

Qualifications

Minimum Education : High School Diploma or GED (Required)

Minimum Work Experience : 3 years Health Information Management experience with emphasis on computer systems and customer service (Required)

Required Skills/Knowledge :

Must be able to work in a fully computerized environment.

Basic understanding of computer applications and working knowledge of Windows Applications and Microsoft Office.

Strong verbal and written communication skills. Strong organization skills and attention to detail.

Maintain confidentiality of patient information.

Ability to problem‑solve and perform in a process‑driven environment.

Strong auditing skills.

Computer keyboard proficiency.

Able to sit for long periods of time at a computer workstation.

Ability to distinguish colors and grades of paper.

Ability to sort alphabetically, sequentially by numbers, dates, and types of documents.

Ability to use judgment when determining appropriateness of involvement of Physicians, Nursing Management and Unit Support Staff when searching for missing documentation or discovering procedural and technical failures.

Must be able to walk and push moderately heavy carts of records throughout hospital facility.

Required Licenses and Certifications : Registered Health Information Technician (RHIT) (Preferred)
