Seeking Home Care Coordinator/CNA + HHA Multiple locations: Chula Vista, Vista, El Cajon, San Ysidro, CA Pay range: $22.39 - $28.09 (depending on experience) Requirements to apply: Active CNA and California Home Health Aide (HHA) certifications REQUIRED CPR and First Aid certification from the American Heart Association At least 1 year of experience working with elderly or frail individuals Strong understanding of HHA scope of practice; works within defined limits Effective team player with clear communication skills Valid California driver’s license required Compassionate Home Care Coordinator needed for Seniors in Southern California – Support, Dignity, and Quality Care Where It Matters Most Job Overview: Under the supervision of the PACE Home Care Coordination RN Manager, the Home Care Coordinator (HHA) supports the Interdisciplinary Team (IDT) by coordinating key home-based services for San Diego PACE participants. Responsibilities include conducting home safety evaluations and arranging IDT-approved services such as meal delivery, caregiver support, and durable medical equipment (DME). This role serves as a compassionate advocate for participants, ensuring care is delivered in a person-centered way that aligns with their goals. The coordinator also collaborates with families, care agencies, and vendors to manage schedules, monitor service quality, and recommend updates to care plans as needed.
