This role involves providing culturally and linguistically appropriate case management services at an assigned high school, in collaboration with Montgomery County Public Schools (MCPS) and the Department of Health and Human Services (DHHS). The goal is to promote self-sufficiency, family development, and connect youth and families to mental health resources, especially targeting un- and under-insured, newly arrived, or at-risk students. Responsibilities include: Providing case management to students and families with social and economic needs. Using electronic record systems to document and update service plans regularly. Maintaining a caseload and coordinating with community partners and service providers. Conducting home visits and participating in workshops and meetings to foster partnerships. Collaborating with therapists, school staff, and community resources while ensuring confidentiality. Requirements: Bachelor’s degree in Social Work, Psychology, or related field; experience serving families can substitute for education. Experience in school-based service delivery preferred. Knowledge of immigrant populations and community resources in Montgomery County preferred. Valid driver’s license with minimal points required. Bilingual fluency in English and Spanish preferred.
