Purpose
TheRegistered Nurse(RN)works in partnership and joint accountabilitywithin an interdisciplinary teamto achieveNeighborcare’sMission,GuidingPrinciples,and goals.The primary focus of the RN is tooptimizethe health status of patientsacross the lifespanthroughchronicconditionmanagement, patient education,emergenttriage, andperforming complex procedures.RNsbuild relationships with their patients through face-to-face and telephonic care andwork in partnership with each other to meet the needs of patients throughoutNeighborcareHealth.TheRN follows the Nursing Processto assess, plan, implement and evaluate nursing carein a Patient Centered Medical Home.

In this position you will:
Working in Partnership and Joint Accountability-10%
Provide care topatientsin the outpatient clinic setting andwork as members ofNeighborcare’snurse team
Maintain joint accountability to the clinic team and to the medicalprogram, toincludesupporting colleagues throughout the organization either on site or remotely as needed
Workwithteam members and leadersto defineand reassess priorities and plan, communicate, and execute care delivery
Participate in Quality and Performance Improvement efforts at the site/programto improve patient outcomes
May serve as a preceptorfornew staff and/orstudents who are assigned toNeighborcareas part of their professional education
Provide community outreach to people who are experiencing homelessness in collaboration with the clinic team and communitypartners.

Establish partnerships with patients and families in person and via telephone for positive outcomes in health status
Identify patient goalsforimproving their health status and management ofchronic physical and mental health conditions
Assess patient understanding of theirconditionand readiness to change
Providepatientswith education about the disease process;offer tools and skills needed to achieve self-management goals
Monitor, track,andfollow-up with patients to ensure care needs are metutilizing patient panel reports
Coordinate carewith outside providersandresources
Apply Neighborcare Health guidelines and standing orders and evidence-based clinicalpractices
Offer medication assisted treatment (MAT): In collaboration with prescribing provider, provide medication assisted treatment (MAT) intake, counseling, guidance, and education to clients experiencing opioid use disorder and other substance use disorders. This includes proactivelyoutreachinga registry of clients for follow up care. This care follows a harm reduction model and uses motivational interviewing to help clients identify and reduce barriers to meeting their treatment goals.
Patient Education-40%
Provide patient education across the lifespan in person and over the phone (select roles may focus on specific patient populations, including pediatric, perinatal, or individuals experiencing homelessness)
Assess barriers to learning and tailor education to meet patients’ individual learning styles and needs
Educate patientsaboutmedications,abnormal laboratoryresults, andself-management of acute and chronic conditions
Promote wellness and the importance of preventive care
Provide motivational interviewing and harm reduction counseling
Triage Emergent Health Issues and Perform Select Clinical Procedures -10%
Respond to emergent health issuesin person andvia telephoneto assess, advise, and provide first aid and Basic Life Support as needed
Perform complex procedures as appropriate for the RN scope of licensure and the individual RN’s competency (may include wound care, IV insertion andfluid/medication administration, urinary catheter insertion and removal)
Administer select injections and medications
Required Qualifications
Completion of an accredited RN program
Va lid Washington State Registered Nurse license
BLS certificate
RN with at least 2 years of triage experience and/or relevant RN experience
Preferred Qualifications
1+ years RN experience in a primary care, hospital and/or home care setting or comparable social service setting
Board Certification in applicable specialty areas
BSN
