Assistant Director of Nursing

Key Responsibilities:
Assist the DON in planning, organizing, and directing all clinical services for home health patients.
Supervise and support RNs, LPNs, and home health aides in the delivery of patient care.
Participate in hiring, training, performance evaluations, and scheduling of clinical staff.
Ensure compliance with regulatory requirements, accreditation standards (e.g., Medicare/Medicaid, Joint Commission), and agency policies.
Conduct case conferences and clinical record reviews to ensure quality care and accurate documentation.
Monitor patient outcomes and implement improvement plans as necessary.
Participate in on-call rotation and provide clinical support during emergencies.
Serve as a liaison between clinical staff, patients, families, physicians, and community partners.
Help develop and implement staff education programs and competency assessments.
Maintain accurate documentation and assist with audits, surveys, and quality assurance activities.
Qualifications:
Current and active RN license in California.
Bachelor of Science in Nursing (BSN) preferred.
Minimum of 2 years of nursing experience, with at least 1 year in home health or a supervisory role.
Knowledge of Medicare Conditions of Participation and OASIS documentation.
Strong leadership, communication, and organizational skills.
Ability to multitask and work in a fast-paced environment.
Proficient in electronic health records (EHR) systems.
Work Environment:
Office and field-based role; may require occasional travel to patient homes.
Flexible scheduling may be required, including evenings or weekends during on-call duties.
