The Social Worker conducts psychosocial assessments and evaluations, as well as provide emotional counseling to patients/families in their place of residence – in person and telephonically. The Social Worker works under the direction of a physician and supervision of the Director of Patient Care Services and serves as a resource to our patients, families and our community. Must be able to utilize computers and be comfortable with electronic medical records. Qualifications Master of Social Work degree from a school accredited or approved by the Council on Social Work Education. Clinical experience relevant to the psychosocial needs of patients and families. Excellent verbal and written communication skills. Spanish speaking a plus, but not required.
