Position Summary: Under the leadership of the Administrator, Nursing Home, the Minimum Data Set (MDS) Coordinator is an active member of the Nursing Home Team that works to ensure that resident care plans are in compliance with established MDS regulations. The MDS Coordinator assesses the medical needs of residents, how well the residents function, and documents the nursing home’s steps taken to support them. The MDS Coordinator completes and submits all the MDS Coordinator paperwork on time and according to established schedules. The MDS ensures that all submissions are accurate and compliant with current regulations and maintains up-to-date knowledge of all proposed and enacted changes to MDS Coordinator regulations.

Basic Qualifications: ·

Experience: Requires a level of experience normally acquired with a minimum of two to four years of progressively more responsible work experience within a long-term care environment or any equivalent combination of education, training and experience. Processes knowledge of MDS rules and regulations including item coding, CAA’S, care planning, electronic submission, scheduling, and has a working knowledge of the relationship between PPS and MDS. ·

Licensure, Registrations & Certifications: Maintains a current state licenses as a Licensed Practical or Registered Nurse and is listed in good standing with the state’s Department of Professional Credentialing.

Essential Job Responsibilities: Ensures that resident care plans are in compliance with established MDS regulations. Completes and submits all the MDS documentation on time and according to established schedules of the Federal and State assessment tool. Acts as an active member of the interdisciplinary and management team providing expertise related to all current and pending MDS regulations. Participates in the planning and adjustment of Medicare assessment scheduling. Ensures that Medicare certifications/re-certifications are completed timely and returned to medical records. Supplies PPS information to biller and other departments. Serves as the Coordinator of care conferences. Ensures that all submissions are accurate and compliant with current regulations. Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive and socialization factors) in planning delivery of care. Maintains comprehensive and up-to-date knowledge of the MDS rules and regulations as described in the MDS User's Manual, including item coding, CAA’s, care planning, electronic submission, scheduling, etc. Committed to an interdisciplinary treatment approach to rehabilitation by exercising sound nursing judgment based on preparation, knowledge, skills, understanding and past experience in nursing care situations. Monitors and ensures compliance and consistent application of the Code of Conduct and Compliance Program, Patient Abuse Reporting and Prevention Policies, and State and Federal Code requirements pertaining to long-term care nursing service. Develops strong relationships and confidence with physicians, administrative team, subordinates and other key stakeholders that creates a culture of positive engagement. · Performs other related job duties as assigned.

Maintains comprehensive and up-to-date knowledge of the MDS rules and regulations as described in the MDS User's Manual, including item coding, CAA’s, care planning, electronic submission, scheduling, etc. Demonstrates confidence and respect of others per performance feedback and/or 360 degree evaluations. Demonstrates on-going performance improvements aimed at operational efficiencies, process improvements and financial outcomes.
