Job Title: Home Health Case Manager – Registered Nurse (RN)

Location: Sacramento, California

Shift: Day Shift | 5x8-Hour (08 : 00–16 : 30)

We are seeking an experienced and dedicated Home Health Case Manager RN to join our team. This role requires a strong background in home health nursing and case management, with a focus on delivering high‑quality, patient‑centered care.

Key Responsibilities

Manage a designated caseload of home health patients, ensuring timely and effective care delivery

Provide direct patient care in accordance with visit productivity standards

Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team

Coordinate care across disciplines to optimize patient outcomes and resource utilization

Monitor visit utilization and manage associated expenses

Participate in agency‑wide orientation, competency evaluations, and provide consultation to clinical staff as needed

Required Qualifications

Active California RN license

BLS certification

Minimum experience in Home Health Case Management
