We are seeking an
ACT Employment Specialist
to join our
Assertive Community Treatment (ACT) Team . Using the evidence-based
Individual Placement and Support (IPS)
model, this role helps adults living with serious mental illness obtain and maintain meaningful employment while supporting their overall recovery goals.
The Employment Specialist works in the
community , providing individualized vocational supports, job coaching, benefits counseling, and follow-along services, while collaborating closely with the interdisciplinary ACT team to ensure integrated care.
Key Responsibilities
Maintain a caseload of 12–15 participants and provide case management
Conduct intake, assessments, treatment planning, and documentation
Develop employer relationships and conduct rapid job searches
Provide individualized job coaching and follow-along support to promote retention
Offer benefits counseling and guidance on employment impact on entitlements
Participate in team meetings, trainings, and crisis support/on-call rotation
Qualifications
Bachelor’s degree in Human Services or related field required
At least 1 year of vocational rehabilitation experience preferred
Ability to work independently and as part of an interdisciplinary team
Excellent communication, organizational, and crisis management skills
Valid NYS driver’s license required
Bilingual (Spanish, Haitian Creole, or Yiddish) a plus
