Flagship Therapy in Santa Rosa, CA is seeking a Full‑Time Physical Therapist for our in‑house rehabilitation program. Our facility provides long‑term, short‑term, and geriatric outpatient services. Join our dedicated team and help drive high‑quality recovery for our geriatric clients.
Base Pay Range $53.00/hr – $63.00/hr (based on experience)
Responsibilities
Screen and evaluate patients with physical functioning disorders and wound‑care needs, developing appropriate care plans that adhere to regulatory and clinical standards.
Deliver rehabilitative, skilled, and medically necessary treatment interventions in accordance with regulatory and clinical practice requirements.
Administer procedures such as manual techniques, ambulation, therapeutic exercises, modalities, and assistive devices; provide wound‑care care.
Consult and counsel patients, families, caregivers, and service providers on physical disorders and rehabilitation progress.
Generate comprehensive discharge summaries that comply with all regulatory and clinical requirements.
Qualifications
Licensed Physical Therapist (required).
Open to therapists at all experience levels; new graduates are welcome.
