Position Summary The Intake Counselor serves as the first point of contact for referral sources and potential patients to schedule assessments and/or admissions.

Responsibilities

Thoroughly assess and complete the assessment tool for all clients.

Communicate information with the on-call provider and nursing to determine an appropriate level of care.

Document all findings and recommendations.

Assist clients in being admitted or referred out to the service or resource to best address their needs.

Manage referrals and ensure prompt response times.

Provide information to individuals, agencies, facilities, and professional staff regarding potential admissions.

Complete insurance verification and pre‑certification as applicable.

Participate in crisis and safety interventions as needed.

Qualifications

Education: Master’s degree in Social Work, Psychology, Counseling or other related field, or RN required.

Experience: At least one year experience utilizing assessment skills preferred.

Age‑Specific Competencies: Adolescent, Adult, Geriatric.

Licensure/Certification: CPR required; CPI Certification required.

HIPAA: Has access to Medical Records of cases to which they are assigned work, for quality compliance and risk related issues. Use of PHI is for purposes of Healthcare Operations.
